New Mandatory Immunization Data Reporting

Beginning January 2013, child care centers and nursery schools will be required to submit the annual school immunization survey data online, via the Online School Assessment Survey (OSAS). The submission of the immunization status of children attending school in the State of New York is mandated by Public Health Law Section 2164. 

To access the online survey, child care center and nursery school program staff members must obtain Health Commerce System (HCS) accounts. The HCS is a free, secure system that is accessible via the Internet -- anytime, anywhere. Up-to-date information on important public health issues and the latest New York State health alerts are also available on the HCS.
