HAS YOUR PROGRAM INFORMATION CHANGED?

To keep our database current, we request child care providers to inform us of any changes in their child care programs. If you have recently changed your contact phone number, for example, please complete the Child Care Provider Information Form and submit it to us by fax, email, or in person.

As long as we have accurate information about child care programs, we can convey that information to families and community members.
